The Resident Experience is Our Primary Focus

In addition to short term rehab to home and long-term care, Brandon Community Care Center is proud to offer specialty services. Our medical director is a geriatric medicine physician with over 40 years’ experience. Two nurse practitioners are on site five days a week. For those requiring special wound care, Brandon Community Care Center’s wound certified physician round once a week and wound care nurses are on site seven days a week.

With a strong therapy department, Brandon Community Care Center not only provides speech therapy, physical therapy and occupational therapy that residents need to go home successfully, but the department also offers restorative therapy to long term care residents so residents are less likely to lose function and ability. In our Ability Rounds, trained staff identify changes in residents that may indicate a decline. These residents are referred to our restorative program. In addition to our in-house rehabilitation services, our therapy department also offers outpatient therapy.

Our services don’t end with quality of care. We also focus on quality of life. Residents and family members often comment on the wide range of activities and our social services.

Visitors to our center may notice groups of students in scrubs. We have the privilege of holding classes that train LPNs and CNAs.

We invite you to tour our center and experience Brandon Community Care Center for yourselves.

Our trained professional care team is committed to providing each individual with quality of life and quality care.

Activities and Amenities

Our communities offer a wide array of activities. We offer trips out in the community as well as a variety of life enrichment activities. Amenities range from salon services and fitness centers, to cafes and libraries. Families, community members, and volunteers are invited to join in social activities.

Who We Are

Established in 1994, CommCare Corporation is a Louisiana-based non-profit that began as owners and operators of skilled nursing and rehabilitation centers. CommCare has since embarked on a journey to provide more integrated, person-centered transitional care throughout the care continuum.

Rehab to Home

While many residents are able to return home, our goal is to get each individual functioning at their highest level and capability through physical therapy, occupational therapy and speech therapy. Our fully equipped rehabilitation gym is staffed by physical therapists, occupational therapists and speech therapists.

Volunteers

Volunteers are integral to the CommCare experience, and volunteering at a CommCare community or hospice can be a rewarding experience! Teach an instrument, voice, or dance? Like to cook? Love to travel? We would love for you to share your passions with our residents and patients. The possibilities are limitless!

Resources

As a non-profit organization, part of our mission is education and outreach. We welcome you to use the resources in our library to get information on topics facing elders and their families. Resources in our library are targeted toward elders, and range from topics that address health and wellness, medical conditions, as well as information concerning Medicare or Medicaid benefits.
