Nestled on a quiet street overlooking a majestic pine stand, Lakeland Community Care Center is known for customized rehabilitation programs based on the specific needs and goals of each resident. Residents and visitors enjoy the well-appointed common areas overlooking landscaped courtyards and views of nature.

Lakeland Community Care Center was awarded a 2024 National Bronze Quality Award based on Baldrige Criteria, the first step in our journey for continuous quality improvement. The Bronze award provides a strong foundation for our quest in earning the Silver Quality Award, for which we have three years to pursue.

Our center provides both short term nursing and rehabilitation for those whose goal is to return home, as well as long term care for those needing nursing care and assistance with activities of daily living.

Rehab to Home

While many residents are able to return home, our goal is to get each individual functioning at their highest level and capability through physical therapy, occupational therapy and speech therapy. Our fully equipped rehabilitation gym is staffed by physical therapists, occupational therapists and speech therapists.
