Culture Change — From Building to Philosophy

Welcome to Trinity Trace Community Care Center. So what is culture change? Culture change is the profession’s term for person-centered care, which honors residents’ choices from dining, to bathing to natural awakenings. Our building is designed to support this philosophy in a form follows function sort of way.

The building comprises a number of smaller more intimate households. Each household is surrounded by patient rooms – all private with the exception of one couples suite–each with their own bathroom. The households have a kitchen, dining room and living area. Rather than having the institutional nursing station at the center of the households, the resident areas are at the center of the households and the caregiver spaces are to the side.

So the building is not the only change for us. Our staff has been involved in ongoing culture change trainings since the fall of last year. As our journey continues, we seek ways to provide care in a different model and we are excited about the direction in which we are moving. We invite you to visit and learn more about our journey.

Whether you or a loved one is seeking long term care or rehabilitation to build the strength, endurance and skills to return home successfully, we understand your concern in making the right choice. Our trained professional care team is committed to providing each individual with quality of life and quality care.

Activities and Amenities

Our communities offer a wide array of activities. We offer trips out in the community as well as a variety of life enrichment activities. Amenities range from salon services and fitness centers, to cafes and libraries. Families, community members, and volunteers are invited to join in social activities.

Who We Are

Established in 1994, CommCare Corporation is a Louisiana-based non-profit that began as owners and operators of skilled nursing and rehabilitation centers. CommCare has since embarked on a journey to provide more integrated, person-centered transitional care throughout the care continuum.

Rehab to Home

While many residents are able to return home, our goal is to get each individual functioning at their highest level and capability through physical therapy, occupational therapy and speech therapy. Our fully equipped rehabilitation gym is staffed by physical therapists, occupational therapists and speech therapists.

Volunteers

Volunteers are integral to the CommCare experience, and volunteering at a CommCare community or hospice can be a rewarding experience! Teach an instrument, voice, or dance? Like to cook? Love to travel? We would love for you to share your passions with our residents and patients. The possibilities are limitless!

Resources

As a non-profit organization, part of our mission is education and outreach. We welcome you to use the resources in our library to get information on topics facing elders and their families. Resources in our library are targeted toward elders, and range from topics that address health and wellness, medical conditions, as well as information concerning Medicare or Medicaid benefits.

A: No. It is licensed by the State of Louisiana as a skilled nursing and rehabilitation center. Trinity Trace Community Care Center offers long term care as well as rehabilitation necessary for individuals to regain strength, endurance and skills to return home successfully.

A: Trinity Trace accepts Medicaid, Medicare, some insurance as well as private payment.

A: Trinity Trace borrows its name from it surroundings. The Trace has long been a lovely natural feature of the Mandeville/Covington area and our center sits right across the street from Most Holy Trinity Catholic Church, from whom the land is leased.

A: Trinity Trace offers physical therapy, occupational therapy and speech therapy that help individuals gain function and ability after an illness, injury or condition. While Trinity Trace provides long term care, many people are able to complete rehabilitation and return home successfully.

A: Trinity Trace offers only skilled nursing and rehabilitation services.
