Culture Change — From Building to Philosophy
Welcome to Trinity Trace Community Care Center. So what is culture change? Culture change is the profession’s term for person-centered care, which honors residents’ choices from dining, to bathing to natural awakenings. Our building is designed to support this philosophy in a form follows function sort of way.
The building comprises a number of smaller more intimate households. Each household is surrounded by patient rooms – all private with the exception of one couples suite–each with their own bathroom. The households have a kitchen, dining room and living area. Rather than having the institutional nursing station at the center of the households, the resident areas are at the center of the households and the caregiver spaces are to the side.
So the building is not the only change for us. Our staff has been involved in ongoing culture change trainings since the fall of last year. As our journey continues, we seek ways to provide care in a different model and we are excited about the direction in which we are moving. We invite you to visit and learn more about our journey.

About Trinity Trace Community Care Center

A: No. It is licensed by the State of Louisiana as a skilled nursing and rehabilitation center. Trinity Trace Community Care Center offers long term care as well as rehabilitation necessary for individuals to regain strength, endurance and skills to return home successfully.
A: Trinity Trace accepts Medicaid, Medicare, some insurance as well as private payment.
A: Trinity Trace borrows its name from it surroundings. The Trace has long been a lovely natural feature of the Mandeville/Covington area and our center sits right across the street from Most Holy Trinity Catholic Church, from whom the land is leased.
A: Trinity Trace offers physical therapy, occupational therapy and speech therapy that help individuals gain function and ability after an illness, injury or condition. While Trinity Trace provides long term care, many people are able to complete rehabilitation and return home successfully.
A: Trinity Trace offers only skilled nursing and rehabilitation services.
